Reset Password

All users in JoinCareTeam are able to reset their own passwords from the login page if forgotten. 

Use these directions if you are unable to login to your account due to a forgotten password. 


1. Open a new Internet browser (Chrome or Mozilla Firefox only) and enter your telehealth portal URL address (e.g.

2. Click on Forgot Your Password underneath the Login button

3. JoinCareTeam will then ask for the email associated with your account - this email is where the password reset link will be sent

4. Enter your email address associated with the account and then click Reset Password  

5. Next, check your email for instructions on resetting your password - email comes from JoinCareTeam

6. Open the email, click on the link provided - you will now be redirected to the password change screen from JoinCareTeam

7. Enter your new password and then confirm your new password on the second line

8. Click Change my Password - your password is reset
